Online Grant Application

The Phoenixville Community Health Foundation accepts only electronic, on-line,  grant requests.   Below is the link to the Grant Request Application.  This link also includes a tutorial.  It is highly recommended that you view the tutorials before beginning your application.

How to access Online Application System

Important Notices

This application process will allow you to save your application and return to it until you formally submit it.  You will not be permitted to submit  your application unless all required fields are completed. The system will not accept applications submitted after the stated grant deadline.

In order to be helpful the following documents will need to be uploaded into the application:

  • IRS Tax Determination Letter
  • PA BCO Certificate- or equivalent
  • Board of Directors that includes leadership role, term, and professional affiliation
  • IRS Form 990 – or equivalent
  • Financial Audit – or equivalent
  • Annual Report
  • Articles of Incorporation
  • By-laws
  • List of public/private funding sources
  • Strategic Plan
  • Current operating budget
